Проведено одноцентровое ретроспективное исследование с участием 230 пациентов с ишемическим инсультом. Оценивали показатели неврологического статуса, сопутствующие заболевания, параметры системы гемостаза, данные антитромботической терапии и групповой принадлежности крови по системе ABO. Прогностическую значимость факторов определяли с использованием ROC-анализа и бинарной логистической регрессии.</p></sec><sec><title>Результаты</title><p>Результаты. Наиболее высокая дискриминационная способность в отношении летального исхода выявлена для шкалы NIHSS (AUC = 0,937). Независимыми предикторами внутрибольничной летальности являлись повышение уровня фибриногена и отсутствие антиагрегантной и антикоагулянтной терапии на догоспитальном этапе. Фибрилляция предсердий была ассоциирована с летальным исходом в однофакторном анализе. Значимых различий по уровню D-димера не выявлено. Связь групп крови системы ABO с внутрибольничной летальностью не установлена.</p></sec><sec><title>Заключение</title><p>Заключение. Полученные данные подтверждают значимость шкалы NIHSS и показателей гемостаза в оценке риска внутрибольничной летальности. Использование клинико-лабораторных параметров позволяет повысить точность прогностической оценки неблагоприятного исхода в стационаре.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>The aim of the research</title><p>The aim of the research. To identify clinical and laboratory predictors of in-hospital mortality in patients with acute ischemic stroke and to assess their prognostic value.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A single-center retrospective study included 230 patients with acute ischemic stroke. Neurological status, comorbid conditions, hemostatic parameters, antithrombotic therapy, and ABO blood group were analyzed. The prognostic value of the studied factors was assessed using ROC analysis and binary logistic regression.</p></sec><sec><title>Results</title><p>Results. The NIHSS score demonstrated the highest discriminative ability for predicting in-hospital mortality (AUC = 0,937). Independent predictors of in-hospital mortality included elevated fibrinogen levels and absence of antiplatelet and anticoagulant therapy at the prehospital stage. Atrial fibrillation was associated with mortality in univariate analysis. No significant association was observed for D-dimer levels. No association was found between ABO blood group and in-hospital mortality.</p></sec><sec><title>Conclusion</title><p>Conclusion. The findings confirm the importance of the NIHSS score and hemostatic parameters for assessing the risk of in-hospital mortality in patients with acute ischemic stroke.
