Оценить влияние маневра прон-позиции у больных с COVID-19 на состояние респираторной системы.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Исследование выполнено у 20 пациентов обоего пола, у которых на фоне СOVID-19 развивалась двухсторонняя внебольничная полисегментарная вирусно-бактериальная пневмония. У всех пациентов выявлялась фоновая и сопутствующая патология: ИБС, сахарный диабет, алиментарно-конституциональное ожирение. Рентгенологическая картина поражения при компьютерной томографии – не менее 75% легочных полей. Больным осуществлялась инвазивная респираторная поддержка аппаратом ИВЛ. Всего было исследовано 8 параметров: Общее ПДКВ (Положительное давления конца выдоха), Авто-ПДКВ (Положительное давления конца выдоха), Сdyn (DynamicCompliance), Cst (StaticCompliance), RE (ExpiratoryResistance), ДО (дыхательный объем), Общий объем выдоха, Остаточный объем. Исследования состояния респираторной системы осуществлялось с помощью Аппарат ИВЛ Neumovent GraphNet Advance.</p></sec><sec><title>Результаты</title><p>Результаты. При оценке группы показателей респираторной системы выявлено снижение Cst у пациентов, находящихся на инвазивной ИВЛ, на 14,7% при p=0,028. Иных изменений параметров не отмечалось.</p></sec><sec><title>Заключение</title><p>Заключение. Установлено, что при выполнении маневра прон-позиции у больных с тяжелым течением COVID-19, находящихся на инвазивной ИВЛ, статический комплайнс снижался на 14,7 %. Величина дыхательного объема, Авто-ПДКВ, объем мертвого пространства и общий дыхательный объем не зависят от маневра прон-позиции при условии полной релаксации пациента.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>The aim of the research</title><p>The aim of the research. To assess the effect of the pron-position maneuver in patients with COVID-19 on the state of the respiratory system.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study was performed in 20 patients of both sexes, who developed bilateral community-acquired polysegmental viral-bacterial pneumonia against the background of COVID-19. All patients had background and concomitant pathology: coronary artery disease, diabetes mellitus, alimentary-constitutional obesity. X-ray picture of the lesion with computed tomography - at least 75% of the pulmonary fields. Patients received invasive respiratory support with a ventilator. A total of 8 parameters were investigated: General PEEP (Positive end expiratory pressure), Auto-PEEP (Positive end expiratory pressure), Сdyn (Dynamic Compliance), Cst (Static Compliance), RE (Expiratory Resistance), DO (tidal volume), Total expiratory volume, Residual volume. The study of the state of the respiratory system was carried out using the Neumovent GraphNet Advance ventilator.</p></sec><sec><title>Results</title><p>Results. When assessing the group of indicators of the respiratory system, a regular decrease in Cst was revealed in patients on invasive mechanical ventilation by 14.7% at p = 0.028. There were no other changes in the parameters.</p></sec><sec><title>Conclusion</title><p>Conclusion. It was found that when performing a pron-position maneuver in patients with a severe course of COVID-19 who are on invasive mechanical ventilation, static compliance decreased by 14.7%.
