Хотя стойкая утрата трудоспособности часто связана с выраженными когнитивными нарушениями, тревожно-депрессивные расстройства (ТДР) также являются распространенным и значимым последствием инсульта. ТДР могут возникать в различные периоды после ОНМК и негативно влияют на реабилитацию, лечение сопутствующих заболеваний и прогноз. Постинсультная депрессия (ПД) затрудняет эффективность реабилитационных мероприятий, уменьшает эффективность лечения сопутствующих заболеваний, утяжеляет и повышает смертность. В статье рассмотрены современные представления о распространенности, этиологии, патогенезе и генетике ПД по данным научной литературы. В патогенезе депрессии важное место занимает обмен биогенных аминов, поэтому особое внимание уделено генетическим факторам нарушения обмена серотонина и его роли в развитии ТДР. В качестве возможных маркеров высокого риска развития депрессии рассмотрены провоспалительные цитокины. В литературе исследования по постинсультной тревоге (ПТ) немногочисленны, особенно по сочетанию ПТ и ПД. Вместе с тем, наличие у пациента с ПД тревоги увеличивает ее продолжительность, степень тяжести, а также значительно снижает эффективность реабилитационных мероприятий.</p></abstract><trans-abstract xml:lang="en"><p>   Disability after store is caused not only by motor, speech, and other focal neurological symptoms but also by neuropsychiatric disorders. Post-stroke depression (PSD) complicates the effectiveness of rehabilitation measures, reduces the efficacy of treatment for concomitant diseases, exacerbates and increases mortality. This article reviews contemporary perspectives on the prevalence, etiology, pathogenesis, and genetics of post-stroke depression based on scientific literature. The pathogenesis of depression is significantly influenced by the metabolism of biogenic amines, thus particular attention is paid to genetic factors of serotonin metabolism disorders and its role in the development of ADD. Pro-inflammatory cytokines are considered as potential markers of high risk for depression development. Literature on post-stroke anxiety (PSA) is scarce, especially regarding the combination of PSA and PSD.
