Разработка дополнительного диагностического критерия раннего выявления надпочечниковой дисфункции, вызванной критическим состоянием (НДВКС) и сопровождающейся сосудистой недостаточностью у пациентов в критическом состоянии при применении экстракорпоральной мембранной оксигенации (ЭКМО).</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проспективное когортное исследование было выполнено в отделении реанимации (29 пациентов на ЭКМО). Пациенты с НДВКС были распределены на следующие группы: Iа (с сосудистой недостаточностью и гипонатриемией) и Iб (с сосудистой недостаточностью без гипонатриемии). В обеих группах проводилось лечение гидрокортизоном при потребности в вазопрессорах ≥0,2 мкг/кг/мин. Индекс кортикорезистентности (ИКР)=536 (нмоль/л) / значение общего кортизола у пациента (нмоль/л).</p></sec><sec><title>Результаты</title><p>Результаты. По результатам сравнительного анализа в группе Ia индекс кортикорезистентности по сравнению с группой Iб в день инициации ЭКМО был выше Ме 2,2-1,2, р=0,03; в последние сутки ЭКМО был ниже – Ме 0,2-0,8, р=0,003. Пациенты группы Iа были старше по возрасту (Ме 58 против Ме 33 лет). В день инициации ЭКМО по шкале SOFA пациенты достоверно значимо не различались. Смертность в группе Iа – 90,9%, в группе Iб – 50%, р&lt;0,05.</p></sec><sec><title>Заключение</title><p>Заключение. Прогностически высокий риск развития кортикорезистентности и высокой летальности наблюдался у пациентов старшей возрастной группы с сосудистой недостаточностью и гипонатриемией при подключении к ЭКМО c индексом кортикорезистентности ≥2,2. Пациенты старшей возрастной группы (Ме 58 лет) нуждаются в назначении гидрокортизона до развития гипонатриемии, увеличения индекса кортикорезистентности ≥2,2, а следовательно, не рекомендовано ориентироваться на потребности в вазопрессорах ≥0,2 мкг/кг/мин.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>The aim of the research</title><p>The aim of the research. Development of an additional diagnostic criterion for the early detection of CIRCI accompanied by vascular insufficiency in critically ill patients using ECMO.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A prospective cohort study was performed in the intensive care unit (29 patients on ECMO). Patients with CIRCI were divided into: Ia (with vascular insufficiency and hyponatremia) and Ib (vascular insufficiency without hyponatremia). Both groups were treated with hydrocortisone when vasopressor requirements were ≥0,2 mcg/kg/min. Corticoresistance index CRI = 536 (nmol/l) / patient’s total cortisol value (nmol/l).</p></sec><sec><title>Results</title><p>Results. According to the results of a comparative analysis, in group Ia the CRI compared to group Ib on the day of initiation of ECMO was higher Me 2,2-1,2, p = 0,03; in the last day ECMO was lower – 0,2-0,8 p = 0,003. Patients in group Ia were older in age (Me 58 vs. 33 years). On the day of ECMO initiation, patients did not differ significantly on the SOFA. Mortality in group Ia is 90,9%, in group Ib – 50%, p &lt;0,05.</p></sec><sec><title>Conclusions</title><p>Conclusions. A prognostically high risk of developing corticoresistance and high mortality was observed in patients of the older age group with vascular insufficiency and hyponatremia when connected to ECMO with a corticoresistance index ≥2,2.
