Длительная резорбция кефалогематомы может сопровождаться формированием хирургических осложнений. Темпы резорбции кефалогематомы имеют неопределенную динамику и могут зависеть от особенностей гемостаза у новорожденных. </p><p>Цель исследования – определить уровень показателей тромбообразования и фибринолиза в плазме венозной крови у новорожденных с кефалогематомами.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Под наблюдением находилось 90 новорожденных, 30 – с кефалогематомами средних и больших размеров (выполняли пункцию кефалогематомы), 30 – с кефалогематомами малых размеров (пункцию не выполняли). Контрольная группа – 30 здоровых новорожденных. Уровень показателей тромбообразования и фибринолиза определяли методом проточной цитофлуориметрии на аппарате Cytoflex LX с использованием наборов для мультиплексного анализа. </p></sec><sec><title>Результаты</title><p>Результаты. На 10-е сутки уровень протромбина у новорожденных первой группы превышал показатели группы контроля в 1,33 раза и в 1,47 раза у второй группы исследования. На 10-е сутки концентрация антитромбина III в первой и второй группах превышала показатели группы контроля в 5,28 и в 8,82 раза, а на 28-е сутки исследования – в 6,9 и 7,28 раза. Концентрация ингибитора активации плазминогена 1 типа в первой и второй группах была выше, чем в группе контроля в 3,11 и 5,25 раза на 10-е сутки, а на 28-е сутки исследования – в 2,88 и 3,93 раза. Уровень D-димера на 10-е сутки в первой и второй группах был выше в 1,43 и 1,71 раза, а на 28-е сутки в обеих группах в 1,6 раза превышал показатели контрольной группы.</p></sec><sec><title>Заключение</title><p>Заключение. Репарация при кефалогематомах сопряжена с механизмами резорбции поднадкостничного кровоизлияния и зависит от фибринолитической активности системы гемостаза. Вероятно, изоляция кефалогематомы от системного кровотока в условиях ингибирования активности плазминогена может являться причиной длительной персистенции поднадкостничного кровоизлияния. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Long-term resorption of cephalohematoma may be accompanied by the formation of surgical complications. The rate of resorption of cephalohematoma has uncertain dynamics and may depend on the characteristics of hemostasis in newborns.</p></sec><sec><title>Aim</title><p>Aim. To define the level of thrombus formation and fibrinolysis in venous blood plasma in newborns with cephalohematomas.</p></sec><sec><title>Methods</title><p>Methods. There were 90 newborns under observation, 30 – with medium and large-sized cephalohematomas (the cephalohematoma was punctured), 30 – with small-sized cephalohematomas (the cephalohematoma was not punctured). The control group – 30 healthy newborns. The level of thrombosis and fibrinolysis indicators was determined by flow cytometry on a Cytoflex LX device using multiplex analysis kits. </p></sec><sec><title>Results</title><p>Results. On the 10th day, the level of prothrombin in newborns of the first group was 1,33 times higher than in the control group, and 1,47 times higher than in the second group of the study. On the 10th day, the concentration of antithrombin III in the first and second groups exceeded the control group by 5.28 and 8,82 times, and on the 28th day of the study by 6,9 and 7,.28 times. The level of D-dimer on the 10th day in the first and second groups was 1,43 and 1,71 times higher, and on the 28th day in both groups it was 1,6 times higher than the control group.</p></sec><sec><title>Conclusion</title><p>Conclusion. Reparation for cephalohematomas is associated with mechanisms of resorption of subperiosteal hemorrhage and depends on the fibrinolytic activity of the hemostatic system.
