Несмотря на проводимую первичную и вторичную профилактику сердечно-сосудистых заболеваний, уровень заболеваемости ишемическим инсультом среди населения Российской Федерации остаётся высоким. Достаточно отлаженная система маршрутизации пациентов с острыми сосудистыми катастрофами и мультидисциплинарный подход в лечении не всегда гарантируют удовлетворительный результат. Широкая распространённость заболевания, высокая степень инвалидизации и смертности, снижение возраста заболевших, экономические потери, связанные с реабилитацией пациентов и длительно сохраняющейся их нетрудоспособностью, диктуют необходимость поиска новых и эффективных подходов в диагностике и лечении данного состояния. </p><p>В настоящее время с целью разработки адъювантной терапии проводятся исследования, направленные на изучение патогенетического каскада реакций, возникающих в момент острой церебральной ишемии. На основании полученных результатов разрабатываются дополнительные возможности в лечении и дальнейшей реабилитации пациентов. </p><p>В представленном литературном обзоре собраны и проанализированы актуальные данные по проблематике ишемического инсульта. Рассмотрена роль отдельных молекул семейства фактора некроза опухолей, принимающих непосредственное участие в развитии и протекании мозгового инфаркта, а также влияющих на дальнейший прогноз выживаемости и восстановления пациентов. </p></abstract><trans-abstract xml:lang="en"><p>The problem of cerebrovascular diseases, in particular acute cerebrovascular accident, does not lose its relevance. Despite the primary and secondary prevention of cardiovascular diseases, the incidence of ischemic stroke among the population of the Russian Federation remains high. A well-established system of routing patients with acute vascular accidents and a multidisciplinary approach to treatment do not always guarantee a satisfactory result. The widespread prevalence of the disease, a high degree of disability and mortality, a decrease in the age of patients, economic losses associated with the rehabilitation of patients and their long-term disability dictate the need to find new and effective approaches to the diagnosis and treatment of this condition.</p><p>Currently conducted studies are aimed at studying the pathogenetic cascade of reactions that occur at the time of acute cerebral ischemia, in order to develop adjuvant therapy. Based on the results obtained, additional options are being developed in the treatment and further rehabilitation of patients. The presented literature review collects and analyzes current data on the problem of ischemic stroke.
