Изучение эффективности расчетных гематологических индексов в прогнозировании риска летального исхода у пациентов с тяжелым течением внебольничной пневмонией, вызванной новой короновирусной инфекцией (COVID-19). </p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведено проспективное обсервационное исследование «случай-контроль» у 450 пациентов с тяжелой внебольничной COVID-19-ассоциированной пневмонией, лечившихся в условиях отделения анестезиологии, реанимации и интенсивной терапии (ОАРИТ). Выполнены расчеты следующих индексов: соотношение нейтрофилов (абс.ч.) к лимфоцитам (абс.ч.) (НЛ), нейтрофилов (%) к альбумину (г/л) (НА), фибриногена (г/л) к альбумину (г/л) (ФА), тромбоцитов (абс.ч.) к лимфоцитам (абс.ч.) (ТЛ), лактатдегидрогеназы (ЛДГ) (ед/л) к лимфоцитам (абс.ч.) (ЛДГЛ), С-реактивного белка (СРБ) (мг/л) к альбумину (г/л) (СРБ-А), ЛДГ (ед/л) к тромбоцитам (абс.ч.) (ЛДГ-Т), ЛДГ (ед/л) к альбумину (г/л) (ЛДГ-А), фибриногена (г/л)*100 к тромбоцитам (абс.ч.*109/л) (ФТ). Индекс системного иммуновоспаления (ИСИВ) рассчитан по формуле: тромбоциты (абс.ч.)*нейтрофилы (абс.ч.)/лимфоциты (абс.ч.). Конечная точка исследования - летальный исход. Статистический анализ проведен с помощью программы SPSS 26.0. Для сравнения прогностической значимости гематологических лабораторных показателей и расчетных индексов в прогнозировании летального исхода проведены однофакторный и многофакторный анализы с построением прогностических моделей с помощью бинарной логист-регрессии. </p></sec><sec><title>Результаты</title><p>Результаты. Определена информативность гематологических индексов по сравнению с гематологическими лабораторными показателями. Наиболее высокую прогностическую чувствительность имеют индексы ЛДГ-Л (78%), ЛДГ-Т (83,9%) и ЛДГ-А (84,4%) с ОШ 3,27, 4,55, 5,58 соответственно. Построены прогностические модели с гематологическими показателями (специфичность  50,7%, чувствительность  88,3%) и расчетными индексами (специфичность  51,1%, чувствительностью 90,8%).  </p></sec><sec><title>Заключение</title><p>Заключение. Расчетные гематологические индексы имеют высокую информативность и могут использоваться для прогнозирования риска летального исхода у больных с тяжелой внебольничной COVID-19-ассоциированной пневмонией. Наибольшая чувствительность и специфичность характерна для гематологических индексов, включающих в расчет соотношение ЛДГ с лимфоцитами, тромбоцитами и альбумином.  </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>The aim of the research</title><p>The aim of the research. To investigate the informativeness of calculated hematological indices in prediction of mortality in patients with severe community-acquired pneumonia caused by new coronavirus infection (COVID-19).  </p></sec><sec><title>Materials and methods</title><p>Materials and methods. We conducted a prospective case-control observational study which included 450 patients with severe community-acquired COVID-19-associated pneumonia treated in the Anesthesiology and Intensive Care Unit (AICU). Several indices were calculated: the ratios of neutrophils (count) to lymphocytes (count) (NL), neutrophils (%) to albumin (g/L) (NA), fibrinogen (g/L) to albumin (g/L) (FA), platelets (count) to lymphocytes (count) (PL), lactate dehydrogenase (LDH) (U/L) to lymphocytes (count) (LDH-L), C-reactive protein (CRP) (mg/L) to albumin (g/L) (CRP-A), LDH (U/L) to platelets (count) (LDH-P), LDH (U/L) to albumin (g/L) (LDH-A), fibrinogen (g/L)*100 to platelets (count) (FP). Systemic immuno-inflammation index (SII) was calculated according to the formula: platelets (count)*neutrophils (count) / lymphocytes (count). The end point was mortality. The statistical analysis was carried out using the SPSS 26.0 program. To compare prognostic value of hematological laboratory parameters and calculated indices univariate and multifactorial analyses were carried out with prognostic models using binary logistic regression. </p></sec><sec><title>Results</title><p>Results. The prognostic value of hematological indices was determined in comparison with hematological laboratory indicators. LDH-L (78%), LDH-P (83.9%) and LDH-A (84.4%) indices had the highest prognostic sensitivity and odds ratio 3.27, 4.55, 5.58, respectively. Prognostic models with hematological indicators (specificity  50.7%, sensitivity  88.3%) and calculated indices (specificity  51.1%, sensitivity 90.8%) were constructed. </p></sec><sec><title>Conclusion</title><p>Conclusion. Calculated hematological indices are highly informative and can be used to predict mortality in patients with severe community-acquired COVID-19-associated pneumonia.
